About This Service
Orthopaedic bracing is an evidence-based adjunct to active rehabilitation - not a substitute. A correctly prescribed brace protects injured tissue during healing, enhances proprioceptive feedback to the nervous system, stabilizes a mechanically incompetent joint, offloads degenerative joint surfaces, and reduces the probability of re-injury during return-to-activity. A poorly chosen or poorly fitted brace does none of those things and may create a false sense of security. Our Registered Physiotherapists and Chiropractor prescribe and fit braces only when the clinical indication is clear and only when paired with an appropriate active rehabilitation program.
Orthopaedic braces fall into four functional categories. Prophylactic braces prevent injury during sport - commonly knee braces for football linemen or ankle braces for basketball. Functional braces stabilize a joint after injury or reconstruction (e.g., functional ACL brace such as the DonJoy Armor or Defiance post-ACL reconstruction). Rehabilitative braces protect surgical repairs while allowing graded range of motion (e.g., hinged post-op knee brace with adjustable ROM after meniscus repair or ACL reconstruction; CAM walker boot after ankle or foot surgery). Unloader braces apply a valgus or varus moment to offload the medial or lateral tibiofemoral compartment in knee osteoarthritis (DonJoy OA Adjuster, Bauerfeind GenuTrain OA) - evidence includes the Raja 2011 and Moyer 2015 systematic reviews. Choice of category, brand, and specific model is driven by the pathology, activity demands, patient anatomy, and budget/insurance constraints.
We fit and prescribe off-the-shelf braces (sized by measurement) for the majority of indications and arrange custom-fabricated braces (cast-molded to the individual limb) for high-level athletes, unusual anatomy, pediatric cases, or when an off-the-shelf model cannot achieve appropriate fit. Lines we work with include DonJoy, Breg, Össur (Rebound, Formfit), Bauerfeind, BioSkin, Shock Doctor, McDavid, Futuro, Medi, and BORT. We have no exclusive contract with any single manufacturer - our brand choice is driven by clinical fit for your specific problem, not vendor preference.
When Should You Get This Treatment?
- ACL / MCL / PCL / LCL injury - functional knee brace during activity (DonJoy Playmaker, DonJoy Armor, Breg Fusion) to stabilize the joint during rehabilitation and return-to-sport.
- Post-ACL reconstruction - hinged rehabilitative brace (DonJoy TROM Adjuster, Breg T Scope Premier) with adjustable ROM locks for the early post-op period.
- Meniscal repair or meniscectomy - post-op hinged brace with ROM restrictions per surgeon protocol.
- Knee osteoarthritis (medial or lateral compartment) - unloader brace (DonJoy OA Adjuster, Bauerfeind GenuTrain OA, Össur Unloader One) for symptomatic offloading, particularly useful for active adults wanting to delay or avoid joint replacement.
- Patellofemoral pain / patellar instability - patellar-tracking brace (DonJoy Tru-Pull Lite, Bauerfeind GenuTrain P3) combined with quadriceps/VMO strengthening.
- Chronic ankle instability or repeat lateral ankle sprains - lace-up ankle brace (Aircast A60, McDavid 195) or semi-rigid stirrup brace for prevention of recurrence.
- Post-ankle fracture or severe sprain - CAM walker boot for controlled weight-bearing during bony or ligamentous healing.
- Lateral epicondylitis (tennis elbow) or medial epicondylitis (golfer's elbow) - counterforce strap (Cho-Pat, Bauerfeind EpiTrain) combined with eccentric loading.
- Carpal tunnel syndrome - neutral wrist cock-up splint worn at night (and during provocative tasks) to maintain the carpal tunnel in a decompressed position - strong first-line evidence (Page 2012 Cochrane).
- De Quervain's tenosynovitis or thumb carpometacarpal OA - thumb spica splint to immobilize the first dorsal compartment or thumb CMC joint.
- Rotator cuff repair or shoulder dislocation - shoulder immobilizer or abduction sling per surgical protocol.
- Lumbar disc herniation, lumbar strain, or lumbar post-surgical support - lumbar support brace (BackOnTrack, Bauerfeind LumboTrain) for limited, targeted use - we do not prescribe long-term lumbar bracing because it can lead to core deconditioning.
- Sacroiliac joint dysfunction, pregnancy-related pelvic girdle pain, or postpartum instability - SI belt (Serola Sacroiliac Belt, Bauerfeind SacroLoc) for mechanical support combined with stabilization exercise.
- Post-concussion athlete returning to contact sport - coordinated helmet-fit and protective equipment review.
- Elderly patients with balance and mobility challenges - prescription and fitting of canes (single-point, quad), walkers (standard, rollator), and other mobility aids.
What to Expect
Prescription visit (30-45 minutes): clinical assessment to confirm the indication for bracing, discussion of realistic expectations (a brace assists healing and stabilization - it does not replace exercise or surgery when those are required), measurement of the limb or torso per manufacturer specifications, and brace selection based on activity demands, budget, and insurance coverage. For complex post-surgical cases we contact the surgeon's office to confirm specific protocol requirements (ROM-locked degrees, weight-bearing restrictions, weaning schedule). For unloader OA braces we discuss the typical 4-6 week adjustment period during which force correction is gradually increased to the therapeutic level.
Fitting and instruction: proper fit is the difference between a brace that works and one that fails. We confirm anatomical alignment, strap tension, hinge placement (for hinged braces), skin protection, and compatibility with your clothing and footwear. We instruct on wear schedule (during activity vs continuous, daytime vs 24-hour, weaning timeline), care (cleaning, skin inspection), and warning signs (skin breakdown, numbness, vascular compromise, increased pain). We schedule a follow-up at 2-4 weeks to reassess fit and symptom response.
Integration with active rehab: bracing is paired with a progressive rehabilitation program because mechanical support alone does not restore muscle strength, neuromuscular control, or proprioception. For ACL cases: quadriceps strengthening, hop progression, neuromuscular control retraining; the brace is worn during return-to-sport, progressively de-emphasized as performance criteria are met. For knee OA: strengthening (GLA:D protocol), weight management education, activity modification; the unloader assists the active program. For carpal tunnel: splint + neural gliding + workplace ergonomic modification.
Custom vs off-the-shelf: most indications are well served by appropriately sized and fitted off-the-shelf braces at significantly lower cost. Custom-fabricated braces are reserved for athletes at high levels, unusual anatomy, pediatric cases, and when appropriate fit cannot otherwise be achieved. We discuss the tradeoffs transparently.
Insurance coverage: most Ontario extended health plans cover orthopaedic braces with a valid prescription from a regulated healthcare provider. Typical limits vary by plan ($300-500 per brace, often per policy year or per device lifetime). We provide: (a) the signed prescription, (b) a medical necessity note documenting the diagnosis and indication, (c) the manufacturer invoice, and (d) a compliant receipt. WSIB covers appropriate braces for accepted workplace injuries. MVA claims may cover braces under SABS if injury-related (OCF-18 approval).
